i think they are not top down but more figure it out yourself - but nobody reconciles the results and decides which method works best and applies that to all - same as covid rules.
but only in the high-paid higher-ups - i think the desk workers you talk to are generally hands-tied and sympathetic, but they have no system in place to suggest ways to improve the methods.
this is what happens when you don't create documentation on how things get done and send it to all provinces to follow.
